  • Overview

    The Patient Care Coordinator (PCC) position provides and coordinates individualized care and resources to assigned MD2U patients. MD2U healthcare services include but are not limited to:  MD2U provider visits, community resources, home health nursing, therapies and social work, laboratory testing, mobile medical imaging, durable medical equipment delivery and teaching, all out-side-the-home testing and transportation. This position monitors the delivery of care by obtaining, reporting and documenting all current information relative to patient care and status to providers and other health care professionals necessary to care for patient.

    POSITION RESPONSIBILITIES

    1. Identify patient issues and determines appropriateness for MD2U’s services. Initiate care plan by coordinating information and care requirements with care providers and resolving any issues.
    2. *Monitor delivery of care by documenting care, identifying progress toward desired care outcomes, and intervening to overcome deviations in the expected plan of care.
    3. *Maintain ongoing communication with care providers regarding variances from the care plan.
    4. *Providing education to others regarding the patient care plan and process.
    5. *Coordinate scheduling of patient appointments with providers and confirm with both patient and provider the date and time once finalized, no later than the end of the business day preceding the visit.
    6. *Process provider orders for patients, determine appropriate level of urgency and expedite orders for prescriptions, required testing and therapies with the appropriate resource.
    7. *Document all activity related to the patients’ care in electronic charting system, including visits, provider orders, contact with additional resources, conversations with patients and other information relative to patient care and well being.
    8. *Answer phones responding to patient and provider concerns, assessing urgency of need, coordinating proper care and/or resources for emergency situations and directing calls to other parties as appropriate.
    9. *Maintain log of all incoming calls, as well as action taken
    10. *Create next day’s schedule in system, updating throughout day to ensure patients due for a visit or those requiring attention are accommodated and providers have the most efficient routes possible.
    11. *Interacting with involved care providers to negotiate and expedite scheduling and completion of tests, procedures, and consults.
    12. *Gather all patient information, including test results and any other documentation required by provider for visit with patient. Ensure timely processing of test results and other pertinent documentation in the EMR prior to the visit.
    13. *Monitor incoming e-mail throughout day and respond within same business day regarding all matters pertaining to patient care and provider concerns.
    14. Supply provider with schedule and directions for patient appointments via email at the end of the day prior appointments.
    15. Compiles weekly a schedule for assigned providers of patients no longer in rotation, but requiring further follow through.
    16. Completes special projects as assigned
    17. Attends Company and industry-related meetings, as required.
    18. Establishes individual goals which are aligned with the Company's business strategies and objectives.
    19. Functions as a contributing member of the department's team and other teams, as assigned.
    20. Performs other duties as assigned by executive leadership.

     

    Qualifications

    1. LPN or Certified MA
    2. Proficient in Microsoft Office products
