The Care Coordinator works collaboratively with a qualified healthcare professional and Patient Care Coordinator to provide comprehensive care for patients including but not limited to: arranging outpatient testing, ordering durable medical equipment, and arranging specialist referrals. Additionally, Care Coordinators will be available to assist their assigned Nurse Practitioners with placing orders as needed, relaying results and changes to plan of care to patients/caregivers and ensuring all information is documented accurately in the patient’s EMR. All time spent on this effort is documented in Aprima Care Management Tool. As a member of the clinical staff they work under the direction and general supervision of a qualified health care professional and are allowed to assist in the performance of specified professional services on their behalf. They also perform certain administrative duties.
