    The Provider Manager will support team growth and retention by planning, directing and coordinating the operations of their care team through developing, monitoring, and maintaining systems and procedures for operating efficiency and best practice patient care. The Manager will also manage staff to optimize performance and patient outcomes.

    Position Responsibilities

    • Monitor and manage territories and patient assignments to optimize efficiency, to include call and time off schedules
    • Foster staff retention through mentoring and communication, to include monthly team meetings that support an open and honest exchange of ideas
    • Consciously create a culture that reflects the practice vision, mission, and goals with a concentration on caring for the frail and elderly
    • Support new staff through regular reviews of their progress, for a minimum of the first 30 days of employment, to include daily telephone or face time contact, discussion of chart reviews, case conferences to discuss difficult patients, and support for increasing the new employee’s patient panel through reassignment of current patients relative to team load, driving distances, etc.
    • Provide a framework for your team that fosters a team oriented atmosphere that will support growth, gold standard clinical care, patient retention, and efficient work practices
    • Provide timely and constructive feedback for staff through routine reviews and ride alongs with providers on the team (first six months then annually afterwards, as well as on an as needed basis)
    • Demonstrate commitment to the team approach to care through pro-active communication with all care partners, including office staff, medical professionals, and families
    • Model professional attention to detail through a thorough understanding of practice processes and policies that includes a careful review and dissemination of information provided through office communications, to include e mail and informational meetings
    • Demonstrate a commitment to excellence through knowledge of best care practices, quality measures, and MD2U practice priorities
    • Develop team goals and requirements based on up to date guidelines, including both national and state specific regulations, maintaining compliance in all areas of practice
    • Maintain an active community presence within the assigned territory; this includes meeting with your marketing support at least monthly, attending marketing and community outreach events and meetings with referral sources, and maintaining an active presence in your market
    • Develop a thorough understanding of practice metrics provided through the Provider Dashboard
    • Produce an annual report for management, to include review of provider retention, market growth, barriers to growth, and a discussion of effective practices (administrative, clinical) used for your team; you will be expected to provide, for review, a log of team meetings, brief notes to chart reviews and communication with new employees, and a discussion of any disciplinary actions and their outcomes (most often will be in conjunction with your annual review)
    • Demonstrate a knowledge of, and willingness to utilize, practice resources, to include clinical management, human resources, financial resources, etc.
    • Manage support services and other clinical resource representatives (DME, pharmaceutical reps, etc.) within the assigned territory
    • Recruitment of new employees, to include ride alongs and interviews with potential candidates and promotion of the practice
    • Maintain supplies for the team by ordering and distributing equipment and marketing materials as needed
    • Demonstrates flexibility to meet the changing demands of a rapidly growing practice


    License Requirements
    • Current CPR certification required.
    • Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
    • Current certification by ANCC or AANP
    • Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.
