Using discretion and independent judgement, the Social Worker serves on the Interdisciplinary Team to assist with management of a patient's hospitalization from pre-admission through post discharge; with particular focus on high acuity/risk patients and families. He/she is an effective communicator and manager of information coordinating healthcare decisions. The Social Worker oversees the effective coordination of services in the area of patient/family advocacy; case communication; conferencing and verbal and written reports. The Social Worker is intimately involved in difficult discharge issues; interfacing with patients and families dealing with difficult end of life issues; patients and families dealing with new life altering diagnosis; chaotic home environments and in all cases involving Elderly/Adult Protective Services. The Social Worker ensures patients' rights are fulfilled and is involved in all grievance issues. He/she also assists patients and families who appeal discharge with Medicare and are issued HINN notifications.
