    Additional Details

    • Assists the patient, significant others, physician, and health care team staff to understand significant personal, emotional, environmental, and social factors and difficulties related to the patients' health problems, which interfere with maximizing the benefit of medical services and the plan of care.
    • Plans for continuity of care with hospitals and community agencies.
    • Assists the patient and significant others to understand, accept, follow, and implement medical recommendations.
    • Assists the patient and significant others in utilizing community resources which will help the patient to achieve and maintain optimal functioning.
    • Visits patient according to plan of treatment. completes a progress note for each visit. and submits progress notes to the agency on an at least weekly basis.
    • Participates in staff conferences and committees as necessary.
    • Fulfills necessary mandatory education on annual basis.
    • Communicates regularly with and carries out only specific duties assigned by the supervising licensed social worker.
    • Updates community resource manual as changes to local resources occur and communicates to staff and patients as appropriate.
    • Communicates information gathered during home visit assessments to the supervising licensed social worker who makes suggestions to the physician for addition to the plan of care.
    • All other duties as assigned.


    Education Requirements

    • Baccalaureate degree in social work, psychology, sociology, or other field related to social work


    Experience Requirements

    • 1 year of social work experience in a health care setting or 2 years of appropriate experience as a social work assistant, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service, except that these determinations of proficiency do not apply with respect to persons initially licensed by a state or seeking initial qualification as a social work assist after December 31, 1977.


    License Requirements

    • Current CPR certification
    • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation


    Additional State Specific Requirements

    • PA: A Baccalaureate degree in social work, psychology, sociology, or other field related to social work or 2 years of social work experience in a health care setting.
