    The RN Field Supervisor / Team Lead for Putnam and surrounding counties.

    Weekly Pay.

     

    Position Summary

    Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records.  May advise patients on health maintenance and disease prevention or provide case management. 

    Additional Details

    Essential Functions:

    • Maintain Accurate, detailed reports and records.
    • Monitor, record and report symptoms and changes in patients' conditions.
    • Record patients' medical information and vital signs.
    • Modify patient treatment plans as indicated by patients' responses and conditions.
    • Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
    • Monitor all aspects of patient care, including diet and physical activity.
    • Direct and supervise less skilled nursing or health care personnel.
    • Observe nurses and visit patients to ensure proper nursing care on a monthly basis.
    • Assess the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems during monthly supervisory visit.
    • Instruct individuals, families and other groups on topics such as health education and disease prevention.
    • Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions.
    • Perform administrative and managerial functions.
    • Provide training or instruction of personnel.
    • Complete patient admissions.
    • Orientate patient and family to process of home care.
    • Interview staff for potential and existing cases.
    • Administer drug screen upon hire and randomly.
    • Assist with job fairs and recruitment.
    • Educate staff on policies and procedures as needed.
    • Complete 90 day and annual performance evaluations.
    • Complete initial and annual CNT skills competency check list.
    • Participate in special projects, meetings, and data review as requested.
    • Attend patient care conferences and supervisory meetings.

     

     

     

    Safety Functions:

    •  Follow policies and procedures related to safety.
    • Conduct visual safety inspections throughout the work day, identify and report unsafe conditions and correct as needed.

    • Immediately report incidents/injuries to supervisor.

    • Incorporate safe behaviors into daily work life.

    • Demonstrate appropriate body mechanics in all situations.

     

    Miscellaneous Functions:

    • Maintain confidentiality of records, conditions, and other information relating to patients, employees, and the company.Work independently, requiring minimal supervision in performance of duties.
    • Demonstrate an awareness of limitations by asking for guidance when needed.
    • Maintain an effective working relationship with all disciplines and promote cooperation among all company team members.
    • Follow and support company philosophy, policies and procedures.
    • Recognize pertinent and/or potential problems and communicate same to appropriate personnel.
    • Demonstrate appropriate behavior when dealing with conflict.
    • Channel information appropriately.
    • Meet company standards and deadlines.
    • Maintain acceptable level of attendance (4-6 absences a year, exclusive Family Medical Leave Act).
    • Willing and available to work as requested.
    • Exercise good judgment in performance of duties.
    • Participate in on-going community awareness effort.
    • Maintain an acceptable level of productivity.
    • Operate motor vehicle as needed for business related activities.
    • Demonstrate commitment to Corporate Compliance Program, knowledge of consequences of non-compliance, and obligation to report possible non-compliance.
    • Working knowledge of state and federal regulations and ACHC requirements.
    • Effectively integrate corporate compliance into planning and daily operations.

    Qualifications

    License, Registration, and / or Certification Requirement: Yes
    Experience Desired
    • A minimum of one year experience as an RN preferred
    License Requirements
    • Must have current RN licensure for state of TN or compact state licensure.
    • Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.
    • Current CPR certification required.
    Skill Requirements
    • Understands the concepts of home health care and the role of the RN as a member of a full discipline health care team.
